1781年，美國獨立戰爭期間。喬治·華盛頓麾下的伊卡博德·克蘭上尉（湯姆·米森 Tom Mison 飾）在戰場上砍掉了一個手執利斧的麵具騎士的頭顱，自己也身負重傷失去意識。當他再度醒來，發現周遭早已翻天覆地，此時他正處在250年後的現代化美國社會。與此同時，斷頭穀治安官被殘忍殺害，他的搭檔艾比·米爾斯（妮可·貝哈瑞 Nicole Beharie 飾）意外邂逅伊卡博德，進而得知殺害治安官的正是陰森的無頭騎士。伊卡博德了解到妻子卡特裏娜（卡蒂亞·溫特 Katia Winter 飾）在他受傷後所做的一切，並從聖經的記載中意識到邪惡的天啟四騎士即將和來自地獄的邪魔軍團蒞臨人間。在此之後，死亡和殺戮在斷頭穀肆意蔓延，女巫、血咒、惡魔，種種鬼魅迷霧縈繞著美國的曆史，從古至今，影響深遠……
